狭义的精神病发病初期，不管症状多轻也仍然是精神病的开始，神经质症的症状不管多重，也仍然是神经质症。神经质症决不会出现反常意识的异常行为。

精神病患者做出异常行为，本人却认为是完全正常的。从客观上看，他们没有自我反省的能力。神经质症患者与此恰恰相反，他们有极强的反省能力。从患者有无克服症状的意识及这种意识的强弱上就可鉴别神经质症与精神病。
